NWP 26-27 Enrollment 2 years old

This form is for children who will be Two Years Old by September 1, 2025.  If you are enrolling twins you will need to do a separate registration for each child.

If you have not already created an account with Planning Center using the Parent Information Form, you will be prompted to add your household information as part of this registration. Note that you will need phone numbers and email addresses of Emergency Contact and Authorized Pick-Up Persons as well as the name and phone number of your child's pediatrician to complete the form.

A non-refundable fee of $200 will be collected upon completion of this form if a classroom space is available.
